Your first visit to Gladden Dental establishes a vital foundation for our relationship and partnership with you. Being well prepared for your first appointment will ensure Dr. Gladden has all the necessary information to provide you with the best possible care.

To expedite your initial visit, new patient forms are provided for you below. Simply click the link below to print and complete the new patient paperwork prior to your first appointment. Remember to bring your dental insurance card if applicable. If for any reason you cannot keep a scheduled appointment, please contact us at least 48 business hours in advance.

Payments

For patient convenience our office gladly accepts cash, personal checks, and major credit cards as payment for services. Payment is expected at the time services are performed.

To assist with extensive dental needs, our office has partnered with Care Credit, a leader in patient healthcare financing that offers multiple ways to conveniently pay for care not covered by insurance and other out-of-pocket costs.

Insurance

As a service to our patients, our office will gladly file claims for your dental benefits. We will also request pretreatment estimates from your insurance provider on the patient’s behalf for more extensive dental needs.
